In order to halt any confusion in it’s tracks… Here are some step by step instructions for you on downloading and filling out our new interactive Emotion Code Session Record forms.

Download File

1.  Right Click (for MACS – CTRL+Click) on this link and Save Download to File. (We recommend putting it on your Desktop or in an easily accessible file folder.)

2.  Install the latest update for Adobe Reader to your computer.  (You will not be able to view this form until this step is complete.)

3.  Open the Session Record Form File. You may want to Right Click (or CTRL+Click) to Open File With Adobe Acrobat or Adobe Reader.

4.  Once you are in the form you will just fill it in as you go! Remember to save the file once you have made changes. You may also print the Session Record Form if you want a more mobile option for working on your clients, but remember to fill the information in on the file saved to your computer later.

5. I would highly recommend watching the video at the top of this page for help filling in these forms.

6. Once you have completed all of your certification requirements, your Session Record form will allow you to email us the completed form. (For instance, if you are missing one client or one of the required fields in the form is incomplete, you will not be able to send the form to us. This helps us to better serve you and get back to you with an evaluation of your forms in a more timely fashion.)

NOTE: As of February 15, we are going to require that anybody getting certified use these new forms.
- These forms are updated in a way that will help lead you through more complete sessions with your clients so we do want everybody to switch over as soon as possible for training purposes.
- If you are currently using the old session record forms and you are almost done with your 30 clients, you can send your completed paperwork in before Feb 15. Just print the new session record form for sessions you haven’t performed yet. Then when you’re ready to send your 30 clients in, instead of sending it as an electronic file, you can fax all of the paperwork together to 1-866-691-9104.
